Indian students wishing to apply to study MBBS in Russia need to meet the following criteria.
1. NEET Qualified
2. Passed class 12 or equivalent examinations with Physics, Chemistry, Biology, and English subjects from a recognised board. OR a high school diploma with a minimum of 50% in Physics, Chemistry, and Biology subjects. For SC/ST/OBC category students, the minimum mark requirement is 45%.

Yes. Most institutes have mentioned a test in Chemistry (class 12 level) for internatioal students. Some colleges may have Russian also as part of the entrance test. While NEET is considered, most colleges ask students to write the test and thn take admissions.
Actually, many colleges do offer MBBS in English for 'indian students. However, Russian is a mandatory subject and all students have to learn it. From the 3rd year on, the course is more Russian oriented because students need to connect and converse with patients in Russian for their internships.
