To apply at the Davao Medical School Foundation College of Medicine, the students must follow the following steps:

• Candidates must fill up the application form (available on the official website of Davao Medical School Foundation College of Medicine)

• The students must attach their educational qualification - College diploma or Certificate of graduation along with transcript of records

• Applicants must attach their NMAT result (minimum score of 40 percentile) along with the required documents. 

• After the evaluation of submitted materials by the Office of the Dean, the status of the application is communicated. DMSF will send the applicant a confirmation letter if they are qualified. 

• The original documents must be authenticated by the Philippine Foreign Service Post (PFSP) in the country of origin and returned to the Office of the Dean.

Local student are required to submit the following documents:

1. Transcript of records - a photocopy of all subjects taken up in college

2. Special Order (original copy)

3. Certificate of Honorable Dismissal (original copy)

4. Certificate of Graduation from Bachelor's Degree from school registraf

5. Certificate of good moral character from two different science teachers during preparatory course

6. Birth Certificate (photocopy)

7. Marriage Contract, if married (duly authenticated by NSO)

8. 2 x 2 ID picture (colored with white background) - 2 photographs

9. 1 x 1 ID picture (colored with white background) - 2 pictures

10. College Diploma - photocopy

11. National Medical Admission Test (NMAT) Result - photocopy

12. Notarized application for CEA with all the requirements therein including fee of P30.00 in postal money order - Pay to: DECS - Region XI (Applicants forms available at DMSFI Registrar's office)

Foreign students seeking admission to the College of Medicine shall submit the following documents in the order outlined below:

1. Application Form and Personal History Statement

Transfer students are required to submit the following documents:

1. Certified True Copy of academic records from the last medical school attended;

2. Certificate of Eligibility to transfer or Honourable Dismissal

3. Photocopy of National Medical Admission Test (NMAT) result

4. Permit from DECS to transfer

Documents required for Student Visa

• Original copy of school's NOA

• Original copy of CEA issued by CHED

• Visa Application form 

• Medical Certificate 

• Chest x-ray, laboratory report and clearance certificate for the AIDS antibody (HIV test)

• Police Clearance from local police authorities where applicant has been permanently residing for the last five (5) years 

• Personal History Statement (PHS) form.

• Guarantee coverage of return fare and other incidental expenses

• Evidence of financial report i.e., bank statements

• Life-size chest x-ray film;

• Eight copies of 2 x 2 coloured photos

• Unexpired passport
