TOEFL is just an english testing exam, itsscore will obviously help you in getting a good college but in selected countries.
You havent mentioned which country you want to study MBBS in. In some countries like Kyrgyzstan, Bangladesh, China you will not be required to give TOEFL scores (you may still give the exam, it will be a choice not a compulsion). For MBBS in the good colleges of USA, clearing TOEFL/IELTS exams is compulsory.
As of 2019 you need to qualify NEET-UG examination and score at least 50% in PCB and English in 12th board to be eligible for mbbs abroad.

As GRE is accepted by majority of colleges to do MS in USA a nd also have english proficiency exams like TOEFL/IELTS to qualify for MS.
So a valid score in both GRE & TOEFL/IELTS is mandatory.
Along with these exams one has to submit some additional requirements thats has been asked.

You can apply in those colleges when their admissions are on. Some of the universities asks for your previous semester's or year's grade cards or mark sheets. and then, later you got to submit your 12th mark sheet once it is out.
SAT,IELTS and all can be attempted any number of times . It's preferable that you start off with SAT exams in your 11th grade itself and then keep on attempting till you achieve your target score. Few books for SAT that can kelp you includes Barrons and Princeton.
