No, it is not compulsory to give USMLE step 1 after completion of 1st year. However, students prefer to give it this way as STEP 1 syllabus consists of Anatomy, Biochemistry, Physiology, Pathology, Pharmacology, Microbiology- subjects which constitute a huge part of 1st year and also 2nd year syllabus. Hence, it will be less work load for STEP 2 and STEP 3 and not much time is needed for revision as you have studied the subjects the last year itself. However, these steps can be taken in any order.
STEP 1 and 2 can be taken in India, but STEP 3 has to be given in the US. So, it completely depends on you, your method of preparation. If you decide to give it after completion of your MBBS, and you take time off for preparation (all the 3 steps) it will take approximately 6 months-1 year. The overall budget should be 10-20 lakh. Foreign medical graduates or FMGs need to qualify the Examination conducted by th e Medical Council of India (MCI). It provides practice certificate to foreign medical graduates if they qualify the FMGE(Foreign Medical Graduates Examination).
It is compulsory for foreign graduates to qualify this exam to start their practice in India.
Though you dont need to appear and qualify the test if you have completed your MBBS degree from Colleges in below mentioned countries-
• MBBS in UK (United Kingdom)
• MBBS in US ( united States)
• MBBS in Canada
• MBBS in New Zealand
• MBBS in Australia
Starting 2023, when the National Exit Test gets implemented, all final-year medical students will need to take it in two steps. All local MBBS students will start internships in the colleges where they graduated. Foreign students successful in the NEXT step 1 exam will have internships in designated sites.
After a mentor-certified internship, they will need to appear for the NEXT step 2 exam and can get a license only after clearing it.

Hi, the eligibility criteria for step 1 and 2 are:
Student is officially enrolled in, or a graduate of, a US or Canadian medical school leading to the MD (Doctor of Medicine) degree. Such US or Canadian medical school should be accredited by the Liaison Committee on Medical Education (LCME) which is an accrediting body for MD programs in the US & Canada and recognized by US Department of Education. [LCME Accredited US or Canadian medical schools].
OR
Student is officially enrolled in, or a graduate of, a US medical school leading to the DO (Doctor of Osteopathic Medicine) degree. Such US medical school should be accredited by American Osteopathic Association (AOA) Commission on Osteopathic College Accreditation (COCA) which is an accrediting body for osteopathic schools, colleges & universities in the US and recognized by US Department of Education. [AOA Accredited US medical schools].
Or
Student is officially enrolled in, or a graduate of, a medical school outside the US and Canada listed in the World Directory of Medical Schools (WDOMS) which meets ECFMG eligibility requirements & criteria. [WDOMS listed medical schools outside US & Canada & meeting ECFMG criteria].
For step 3:
Student holds MD (or its equivalent; like MBBS) degree from LCME Accredited US or Canadian medical school OR ; Student holds DO degree from AOA Accredited US medical school OR ; Student holds MD (or its equivalent; like MBBS) degree from WDOMS listed medical school outside the US & Canada & possess ECFMG Certification.
And
Student has passed USMLE Step 1 and USMLE Step 2 [Clinical Skills (CS) as well as Clinical Knowledge (CK)]. Hope this helps.
Hi, the information you gave is insufficient. Are you planning for a PhD or are you planning for a job in USA. As the job visa are hard to get (not impossible). For a PhD there are good universities that you can get into are Johns Hopkins University, Emory School of medicines, etc. If you still have doubts feel free to comment here.

when it comes to compare them, there is no comparison between them as they both are same and totally the choice depends on you. If you want to complete your medical practice in India then go for NEET PG exam . But if you want to practice in the USA ,then you should go for USMLE. After USMLE without any licensing exams, you can work in many countries including India without any licensing exams.
